Medical Clearance for Clinical Rotations
If you would like to get your medical clearance for your clinical rotations at a Northwell Health-GoHealth Urgent Care Center, please fill out this form. Medical clearance visits can only be requested through this form and should be requested more than 72 hours in advance of your preferred visit time. If you need urgent care services, please save your spot above at Forest Hills, Bulls Head, or one of our other 50+ centers.
*Please note that medical clearance visits are not covered through insurance in the urgent care setting and the cost is $200. Medical clearance visits will include a physical examination, a two-step PPD test, and flu shot. Chest x-rays are available for an additional $95*
